By submitting this intake form, you acknowledge and agree to the standard operational parameters setforth by Integrity Screening & Compliance LLC:

  • Cancellation & Late Arrival Policy: Late arrivals exceeding 10 minutes past scheduled windows are subject to a $35.00 late penalty fee. Cancellations submitted with less than 24 hours' notice are billed at 100% of the base fee for testing.


  • Shy Bladder Protocol: Donor collections requiring extended hydration monitoring beyond standard windows incur the cost of a new collection plus a $50.00 surcharge for collector supervision.


  • Data Privacy: Client and candidate data is securely stored and processed in accordance with applicable federal and state privacy regulations.


  • Emergency Services Fee: Service requests requiring immediate or on-demand dispatch within 2 hours or less are subject to an additional emergency service fee.
